What to Bring to Your Appointment
- Distance, computer, and reading glasses, as well as sunglasses
- Current contact lens boxes and/or blister packs, if applicable
- An up-to-date list of medications
- A valid form of identification, such as a driver's license
- Vision and medical insurance cards
- New patients: if available, please have a copy of your most recent eye exam records sent to our office prior to your visit
